Make An Appointment
First time at Evolve? You are required to book an Initial Appointment for Physiotherapy. Click the button above then follow the instructions, It’s easy! Payment will be required to confirm your booking. If you have a health fund we can send you the receipts after the consultation for rebate.
IMPORTANT: If you’re an existing client, ensure you use your booking code (found on any previous booking confirmation email) to avoid creating duplicate profile. If you cant find your code please text or call admin to receive.
Private Health Insurance If you would like to claim through HICAPS at the time of your appointment. you will need to call the clinic and have admin make the appointment. Make sure you bring your card with you (physical or digital) then you will only need to pay the gap.
If you have a Medicare EPC referral make sure you bring the referral and your Medicare card. We can claim the Medicare rebate for you at the time of the appointment. Please note you will need a debit card (Savings or Cheque account) for us to process the rebate for you.
If you are a Workers Compensation or Third Party patient please call and inform admin prior to your appointment with details of your doctor’s referral, claim number and case manager name. We will look after the rest.
